It seems that Dynamic Widgets 1.5.0 let widget options sometimes disappear when you add a widget to a sidebar. Only the Save button remains. Just hit that Save button and all options will reappear.
I’m going to look into it.
UPDATE: [13 Jan 2012] Development Version 1.5.0.1 has a fix for this issue.

I had/have the same problem. Thought it was my browser. Then I thought it was my theme. Then I realized that it was dynamic widgets, and I found the error mentioned on the developer’s website (https://qurl.nl/dynamic-widgets/issue-tracker/). Although I didn’t see a public solution, just that they reached a solution.
I might have an idea where the error came from. I logged into my website the other day, and I saw a code error. I found the error in Dynamic Widgets (custom_post or something like that), where there were a lot of extra (&s) in the code. I guess this had to do with a code problem between the updated WordPress and Dynamic Widgets plugin. After I took out the (&s) the error went away, and I had this problem with widget text areas disappearing.
So I guess it seems that the problem was the developer’s codes matching up with the newest WordPress update. It seems that the devs have found the problem, and there will probably be an update soon. (Although they DID just update 4 days ago, this shows that the devs are on it, and this could also go to show that this plugin, overall, is very reliable)

I did indeed found a solution for the disappearing options and the taxonomy warning. Both these fixes have already been committed to development version 1.5.0.1 (see link at the right, in the sidebar).
The pass-by reference warning looks to be fixed in my development environment. When I’m (pretty) sure it is, I’ll commit that also to the next development version.

Not sure if your widget is to blame, but since updating to 1.5.0, I have lost all text widgets TWICE! A real bummer. I had to get my host to restore my site the first time.
Not sure how it happened except it began directly after updating WordPress and the Dynamic widget. The second time, I had a blank text window open on the widgets page when I went to make another one Dynamic. When it return, all text widgets had disappeared.
I need to find where my widgets are store on my server and make a backup, re-creating them is very time consuming. Cheers from Australia

Be sure to hit Save before going to the DW edit page. DW does not interfere with the widgets placing and content admin, other than adding a link to the DW edit page.

Three times now, erased every text widget, BUT I have discovered it is always when uploading the same content – which I have found is UNICODE text. Changed it to ANSI text and it went through OK…… OMG
I’ll try another of the same bred. Fingers Crossed.

That might indicate an encoding problem within the database table(s) or with the database connection.
